*Every person who is attending needs to sign a waiver form. If there are multiple people in your party, please have them complete their own form (available on the event info page).

Please read the waiver form below carefully and complete the required information.

1. I am the legal owner or agent for the owner of the dog(s) named above.

2. My dog is in good health, is up to date on all appropriate vaccinations, is free of fleas/lice and has not been ill with any known contagious virus within the last 14 days.

3. I understand that even with vaccines there is a small risk that my dog(s) may contract a contagious disease or illness. I agree that should this occur I am responsible for my dog(s) care and treatment and all associated costs.

4. I agree and understand that my attendance at Tollermania is not without risk to myself, my dog(s), other family members or guests, and other event participants (human or canine). 

5. Such risks to myself and/or my dog include, but are not limited to; death, personal injury, physical injury, mental injury, emotional distress, trauma, loss, disappearance, theft, expenses, loss of income and property damage.

6. All risks are accepted by me.

7. I agree that, as owner of the above dog(s), I am solely responsible for the dog(s) behavior and physical control at all times.

8. I understand that although dogs are supervised, incidents or injuries may occur from playing with other dogs, which include but are not limited to; bites, scrapes, scratches, and sprains. I assume all liability for any such injury to myself or my dog(s).

9. By reading, signing and dating this document, I hereby waive and release Daniel Barnhill and/or his agents or representatives and/or other participants in Tollermania from any and all damages, losses, fines, claims, expenses (including all legal costs), judgments and/or liabilities of any form or nature resulting from any act or omission of any dog, dog owner or guest, participating in Tollermania.

10. I understand that I am attending Tollermania as a guest of Daniel Barnhill and he or his representative or agent, has the right to remove either myself, my guest(s) and/or my dog(s) from the Tollermania premises for any reason and at any time. I will accept all costs and losses caused by said removal.

By typing my name below, I understand and agree that this form of electronic signature has the same legal force and effect as a manual signature.
